Columbus Health System – 100% Outpatient, No Call, and Excellent

Internal Medicine or Family Medicine Physician to join a rapidly
growing and well-regarded healthcare system located just 20 minutes
from Franklin County, Ohio. This is an exceptional opportunity for
a dedicated primary care provider who is passionate about
team-based care and making a meaningful impact on population health
outcomes. Position Overview: - Setting: 100% Outpatient - Schedule:
Monday – Friday, 8:00 AM – 5:00 PM - Call: Light call schedule
(1:3), no weekends - Patient Population: Adults with chronic and
complex conditions - Team-Based Care: Collaborate closely with
social work, case management, behavioral health, pharmacy, and
nutrition - Care Coordination: Monitor hospital/ER discharges, SNF
transitions, and home care - Focus: Prevent avoidable ER visits,
hospitalizations, and readmissions through proactive disease
management Requirements - Board Certified in Internal Medicine or
Family Medicine - Active Ohio medical license (preferred but not
required for application) - BLS Certification - Experience in
population health management strongly preferred - Strong
communication and coordination skills in a multidisciplinary care
