Job Summary: The Clinical Outreach Provider (NP/PA) delivers
high-quality primary care across diverse community settings,
including school-based health centers (serving K–12), homeless
shelters, outpatient treatment centers, and other outreach
locations. This role is designed for a nurse practitioner or
physician assistant with a background in family medicine or
internal medicine. Pediatric or substance use management experience
is preferred but not required. The provider will offer clinical
services both within Care Alliance’s brick-and-mortar clinics and
through mobile medical units, ensuring access to care for
underserved and vulnerable populations. The Clinical Outreach
Provider will practice within an integrated, team-based model of
care, collaborating with physicians, behavioral health clinicians,
pharmacists, case managers, and community partners. Requirements:
Competencies/Responsibilities: *Include but are not limited to:
Clinical Care · Provide comprehensive primary care services across
the lifespan, including preventive, acute, and chronic disease
management. · Deliver care at outreach sites, including K–12
school-based health centers, homeless shelters, recovery programs,
and other designated locations. · Diagnose, treat, and manage acute
and chronic conditions according to evidence-based guidelines. ·
Prescribe medications, order diagnostic tests, and develop
individualized care plans. · Provide immunizations, screenings, and
age-appropriate preventive services. · Support patients in
accessing specialty care, behavioral health services, and social
support resources. Outreach & Integration · Serve as a key provider
on Care Alliance’s mobile medical units and community-based
clinics. · Collaborate with schools, shelters, recovery centers,
and community partners to meet health needs in non-traditional
settings. · Deliver care that is culturally competent,
trauma-informed, and responsive to the needs of diverse
populations. · Actively participate in care coordination with
interdisciplinary teams to ensure continuity of care across
outreach and clinic-based settings. Compliance & Quality · Document
all encounters in the electronic medical record (EMR) in a timely
and accurate manner. · Follow Care Alliance protocols, state and
federal regulations, and evidence-based practice standards. ·
Participate in quality improvement activities, population health
initiatives, and program evaluation. · Contribute to organizational
goals around access, equity, and improved patient outcomes. Minimum
Education and Experience: Required: · Licensed Nurse Practitioner
(NP) or Physician Assistant (PA) in the State of Ohio. ·
Board-certified in Family Medicine, Internal Medicine, or related
specialty. · Pediatric or substance use management experience
preferred but not required. · Current DEA license. · Two or more
years of clinical practice in primary care or community health
strongly preferred. · Commitment to serving diverse, underserved
populations with compassion and respect. · Strong collaboration,
communication, and problem-solving skills. · Ability to work in
both traditional clinical environments and mobile/outreach
settings.
